Job Description
Responsibilities*
The U-M School of Nursing is seeking a Health Coach. The position is aligned with the HealthyLifetime research program, which is a 5-month personalized health coaching program for adults, 30 years of age or older, that includes a comprehensive assessment made through surveys and a virtual home assessment, a goal-directed strengthening action plan developed by the participant with the guidance of the nurse, and a tailored daily planning guide to enhance action steps and goal attainment. The program consists of an 8-week "intensive" period wich includes 8-weekly coaching sessions with the coach (two, hour-long initial sessions and 6 weekly 30-minute sessions) and is followed by three additional months in which the client can request up to three additional 30-minute sessions (for a total of five months). All sessions are done vitually using video technology via a computer.
The purpose of the HealthyLifetime [HL] research study is to assess changes in participants' health outcomes after the 8-week intensive phase and the sustainability of outcomes at five months. Additionally, the investigative team evaluates the relationship of level of participant program engagement [e.g. number of video sessions attended] to magnitude of change in outcome measures.
POSITION PURPOSE AND RESPONSIBILITY
The Health Coach promotes wellness, resiliency, and independence by guiding clients to establish their vision for their long-term health and independence, identify lifestyle changes that support their vision, establish goals and strategies for healthier lifestyle behaviors considerate of their unique life circumstances, and strengthen their motivation, agency, and confidence to acheive their goals. The Health Coach will be responsible for delivering the HL program to their assigned clients.
